To assist the medical referee in approving this form please note:

  1. If you have not attended the deceased within 14 days of their death in the community, the Coroner MUST be informed
  2. If the patient died in hospital within 24 hours of admission, the Kent and Medway coroners no longer require automatic referral. However, if there is any uncertainty about the events prior to, or the cause, of death, you MUST still refer to the coroner. If the death occurs on the background of an explanatory illness, or there is an obvious natural cause, referral is not required. The patient’s GP can often help provide additional information, which may help you. If ever you are in doubt, you should refer to the coroners’ office.
  3. Question 9 requires a description of the events immediately before death, NOT just the cause of death.
  4. Please provide your mobile as a contact number rather than just the hospital switch board or a bleep number, so you can be readily contacted, if needed. This helps reduce potentially distressing and unnecessary delays for families

This form may only be completed by a registered medical practitioner of at least five years’ standing who is not either a relative of the deceased, the medical practitioner who issued the medical certificate (form Cremation 4) or a relative or a partner or colleague in the same practice or clinical team as the medical practitioner who issued that certificate.

‘Five years’ standing’ means a medical practitioner who has been a fully registered person within the meaning of the Medical Act 1983 for at least five years and, if paragraph 10 of Schedule 1 to the Medical Act 1983 (Amendment) Order 2002 (S.I. 2002/3135) has come into force, has held a licence to practice for at least five years or since the coming into force of that paragraph.
